ADDRESSING A GAP IN OUR HEALTH SYSTEM

The adage “necessity is the mother of invention” can describe the story of the founders of eFamilyCare collaborating to create the first-ever digital platform to connect family caregivers of chronically ill loved ones with anytime, anywhere access to experts.

The gap in the healthcare system that the founders identified was in providing support to the 53 million family caregivers in the United States. With the senior population only growing and with the healthcare industry constantly pushing care to the home, family caregivers need guidance. Combining their experience in technology, healthcare management and caregiving led to the creation of eFamilyCare.

CLINICIANS AND INNOVATORS PASSIONATE ABOUT IMPROVING HEALTH

As president of NYU Hospitals Center, Dr. Eric Rackow saw first-hand the need to better support family caregivers of patients who were unnecessarily experiencing frequent hospitalizations. His insights led him to lead SeniorBridge and to the development of a unique model of complex chronic care management and homecare nationwide. As evidence mounted about the opportunity to profoundly improve health, quality of life and senior care, SeniorBridge was ultimately acquired by Humana, and became known as Humana At Home, a nationwide care management company.

Eric has developed and led large health care organizations, taken care of thousands of patients in their homes, and has hands-on experience in taking care of patients.

His deep understanding and experience are immeasurable, and his learnings have now been integrated into a virtual care model, the first ever of its kind, provided in a mobile, convenient and low-cost service.

Eric Rackow, MD, Executive Chairman and Co-Founder

Eric C. Rackow, M.D. is a Professor of Medicine at the New York University School of Medicine. He is the immediate past president of Humana At Home (2012-2017), and was responsible for the development, growth and success of this Humana division. Prior to joining Humana, he was president and CEO of SeniorBridge (2007-2012), where under his leadership the company tripled in size to over 50 homecare offices and a nationwide network of more than 10,000 care managers coast to coast. Humana acquired the company in 2012 (NYSE:HUM). Dr. Rackow joined SeniorBridge following a career in the hospital industry where he saw first-hand the challenges frail seniors face in their homes, and the need to provide personalized, ongoing support. There he became passionate about the need of averting unnecessary hospitalizations and emergency room visits through care coordination at home. Dr. Rackow is the author of 184 articles and 40 chapters on healthcare delivery and the care of patients with complex medical problems.

Dr. Rackow’s hospital leadership include past president of NYU Hospitals Center (2004-2007), where he was responsible for ensuring the quality of medical services and promoting continued excellence in patient care, medical education, and clinical research. Previously, he served as Chief Medical Officer at NYU Hospitals Center (2000-2003) and Chairman of the Department of Medicine at St. Vincent’s Hospital and Medical Center of New York (1989-1999). After earning his MD at the State University of New York, Downstate Medical Center, he trained in Internal Medicine, served as Chief Resident in Internal Medicine and completed a fellowship in Cardiology at Downstate. He is on the Board of Board of Trustees of his alma mater, Franklin and Marshall College. Dr. Rackow is a fellow of the American College of Physicians, American College of Critical Care Medicine, American College of Cardiology, and American College of Chest Physicians. In view of his outstanding contributions to healthcare, in 2004 Dr. Rackow received the distinguished award of Mastership of the American College of Physicians.

Dr. John Rowe is the Julius B. Richmond Professor of Health Policy and Aging at the Columbia University Mailman School of Public Health. Previously, from 2000 until his retirement in late 2006, Dr. Rowe served as Chairman and CEO of Aetna, Inc., one of the nation’s leading health care and related benefits organizations. Before his tenure at Aetna, from 1988 to 2000, Dr. Rowe served as President and Chief Executive Officer of Mount Sinai NYU Health, one of the nation’s largest academic health care organizations. From 1988 to 1998, prior to the Mount Sinai-NYU Health merger, Dr. Rowe was President of the Mount Sinai Hospital and the Mount Sinai School of Medicine in New York City.

Before joining Mount Sinai, Dr. Rowe was a Professor of Medicine and the founding Director of the Division on Aging at the Harvard Medical School, as well as Chief of Gerontology at Boston’s Beth Israel Hospital. Dr. Rowe has received many honors and awards for his research and health policy efforts regarding care of the elderly. Currently, Dr. Rowe leads the Research Network on An Aging Society.

Dr. Rowe is a Fellow of the American Academy of Arts and Sciences and a member of the National Academy of Medicine of the National Academy of Sciences. He. served on the Board of Trustees of the Rockefeller Foundation, and was Chair of the Board of Fellows of Harvard Medical School and the Board of Trustees at the Marine Biological Laboratory in Woods Hole, Massachusetts. Dr. Rowe is a former Commissioner of the Medicare Payment Advisory Commission (MedPAC).

Dr Rowe has received many awards and recognitions in business, including being listed in the top ten managers of the year, in the world, by Business Week in 2005 and being included in Harvard Business Review’s 2013 listing of the best performing CEOs in the world, in all industries.

Larry Sosnow has over 40 years of experience in home health care. His career has focused on the founding and development of disruptive health care service companies. He founded and was Chairman & Chief Executive Officer of Patient Care, Inc., a home healthcare company, from 1975 to 1995, when it was acquired by Chemed (NYSE:CHE). He founded and was Chairman of SeniorBridge Family Companies Inc., a home health care management company, from 2000 to 2012, when it was acquired by Humana (NYSE:HUM). During this time, he also cofounded and served as Vice Chairman of Athena Health Inc., an electronic health records company and helped guide guide the company’s focus toward medical practice management (NASDAQ:ATHN) from 1997 to 2004. Mr. Sosnow was a Founding Member of the National Association for Home Care in 1980 and served on its Executive Committee and Board. He has also served on standard-setting legislative and licensing committees, such as the New York State Health Department’s Licensure Committee and New Jersey’s Home Health Agency Assembly. In 2015, Larry co-founded the Solomon Leadership Program mentoring teenage leaders. Larry obtained his BA in Business Administration from Upsala College.

Ernie Pomerantz has more than 50 years of experience investing in both public and private companies in a broad range of industries and has served as a board member of more than forty companies. He spent nine years at Kuhn, Loeb & Co and twenty-two years at Warburg Pincus where he was an investment banker, analyst, portfolio manager and private equity investor. Following his twenty-two years at Warburg Pincus, he was a partner at MESA Partners and then Chairman of StoneWater Capital LLC until its acquisition. In addition to eFamilyCare, Ernie served on the boards of HelpCare Inc. (acquired by HealthEquity Inc.,) PingMD Inc., and MedicaMetrix, Inc. in the healthcare industry. His investments and directorships have been focused on early-stage companies. Ernie served as a Lieutenant in The United States Navy from 1964 to 1969. In his role at IDE Corp., the leader in K-12 educational consulting and online professional learning services, he led a nationwide expansion and developed partnerships with educational technology industry leaders. Boyd obtained his Bachelor of Arts from SUNY at Buffalo, a Masters in Teaching from Fordham University, and a Masters in Information Systems Management from Marist College.

Jay S. Weinstein M.D., M.B.A is currently an Assistant Professor of Medicine at the Zucker School of Medicine at Hofstra/Northwell and co-medical director of the primary care practice for Northwell Health. He is a supervising medical attending in their internal medicine residency and a practicing internal medicine physician for Northwell Health. Dr. Weinstein has over twenty-five years of clinical and teaching experience in internal medicine. Prior to joining Northwell, he was a faculty and teaching physician for Continuum Health System and a former medical director and faculty physician for St. Vincent’s Hospital and Medical Center in Manhattan. He was the Director of the Transitional Residency Program and Medical Director of the Medical Consultative Service. Dr Weinstein received his M.B.A. in 2018 from the Zarb School of Business at Hofstra University, graduating with distinction. Dr. Weinstein has an interest in preventive medicine, quality of care and the cost of healthcare.

Irene Hernandez, Director of Partnerships

Irene Hernandez earned her Bachelor of Science in Psychology and Biology from Florida State University, while also working as an EMT at Tallahassee Memorial Hospital. She continued working as an EMT for 3 years before diving into a career in financial services and business development for 10 years. Irene’s passion for healthcare and extensive experience in enterprise sales led her to her role at eFamilyCare. She is also extremely passionate about being involved in the community and has created her own community group called Broward Dachshunds, a group that expanded from 11 members to close to 3,000. She has raised money for rescue organizations as well as provided awareness, support, and resources to her members so that they are better equipped when raising this special breed.

Karen Katter, MPA, Chief Operating Officer

Karen Katter is health care leader with extensive experience in Medicare and Medicaid managed care operations and geriatric care management. Karen also has a deep background in network development, contract oversight and cross functional team building. Most recently, Karen led a national network of care managers at Humana At Home who delivered innovative and cost effective care to vulnerable seniors. She earned her Bachelor of Arts from Boston University and a Master of Public Administration from New York University.

She served as an Executive Vice President of SeniorBridge Family Companies and subsequently Chief of Professional Services at Human At Home after Humana’s acquisition of SeniorBridge in 2012. Ms. Fine has worked in the field of eldercare for over 30 years and was a founding partner with the New York City based Fine & Newcombe Associates, a pioneering firm in the private eldercare field that was acquired by SeniorBridge in 2000. She is the former President of the National Association of Professional Geriatric Care Managers. She is the recipient of the prestigious Hunter College School of Social Work Award for Outstanding Service to Social Work in Aging. Claudia obtained her BA at Brandeis University and her Master of Social Work at the Hunter College and Master of Public Health from Columbia University.

David Zychowski is a sales executive with 22+ years of experience as a sales leader in the healthcare industry. He has led award winning sales teams in wound care biologics, nurse practitioner services, short term rehab, long term care, acute care and homecare before turning his attention to supporting the family caregiver. David spent the last 11 years at Humana at Home/SeniorBridge where he first worked with the eFamilyCare senior leadership team, supporting chronically ill, clinically complex clients at home.

During his recent role at SeniorBridge, David led various teams around the country and worked collaboratively with sales, operations, and clinical personnel to exceed expectations through three years of constant challenge and change. During this time, he also became a Humana Mentor to several peers and new leaders outside the sales organization.

David received his Bachelor of Arts degree from the University of Connecticut and honed his leadership skills at The Center for Creative Leadership, Sandler Institute, and Leader Activator Standard.
